Pre-tax dollars beat any rebate

Before you optimize the card, optimize the account. A dollar spent from an HSA or FSA is a pre-tax dollar; depending on your bracket that's effectively a discount far larger than any 2% rebate a credit card will ever pay. If the expense is HSA- or FSA-eligible and you have the balance, paying from that account is the higher-value move and the card question is moot. Rewards only come into play for what's left over.

Ask about the provider's own payment plan first

Hospitals and clinics very often offer interest-free payment plans, and many will reduce a bill on request or apply financial assistance you have to ask for. An interest-free plan beats a rewards card you'd carry a balance on by an enormous margin — a 2% rebate against 20%-plus APR is not a close contest. Put a medical bill on a card for the rewards only if you can clear it in full that month.

Watch for the surcharge

Some providers and billing services add a card processing fee, typically 2% to 3%. Where they do, paying by card costs more than it earns and the answer is bank transfer or check. As with every other bill on this site, the biller's payment page tells you in one line what no ranking can.
